Occupational Health Questionnaire

Now that you have been given an offer of a place to study at London South Bank University, the University needs to be aware of any disabilities or health conditions that could be relevant to your proposed course of training and future employment. Pre-course health clearance is undertaken by us, OHWorks Ltd, the occupational health service for London South Bank University. Where considered appropriate, we can then advise your chosen School of the need to consider any reasonable adjustments or additional support needs.

The University is committed to providing equality of opportunity for disabled students and where possible all reasonable support will be provided to enable them to complete the course. For those undertaking healthcare studies or teacher training, the University needs to ensure that all applicants will be able to fulfil the requirements of the relevant regulatory body in meeting the core competencies and, following graduation, be medically suitable to work within their chosen field. In the rare case that it is decided that you are medically unsuitable for the course the University will endeavour to advise you about suitable alternative courses.

You can be assured that the information will remain confidential to the staff of OHWorks. The University will only be informed of the functional effects of any health problems/disability if this is relevant to your educational needs or student/patient safety and of the need to consider reasonable adjustments and/or additional support. Specific medical information is not usually disclosed unless we have your full consent.

The University has agreed to pay for all your occupational health appointments, immunisations and blood tests. For invoicing purposes OHWorks needs to inform the university of the blood tests (not the results) and immunisations you have had, as well as the dates of your occupational health appointments and whether you attended or failed to attend.

By completing this online questionnaire you are consenting to information being held and processed by OHWorks Ltd as the occupational health provider to London South Bank University. The information provided online will form the basis of your occupational health record.

We are required to process your health questionnaire within a short period of time. If we do not process your health questionnaire in time you will not be cleared to start your course. We therefore ask you do the following:

  • Complete this form immediately; don’t delay as we may need more information from you, or your GP, which may delay things further
  • Check your emails regularly as we may contact you by email (including your Spam/Junk emails)
  • Ensure you are available on your mobile phone as we contact many students to ask further questions. This may be during the day, evenings or weekends

If we have made contact with you and you do not respond to us within 5 working days we will advise the University that you have not responded to our request for further information. We will not undertake any further follow up which may affect your place on the course.
